When you have your newly assigned social security number, do two things:

to go over your tax requirements, if any. Please bring the
following with you:

-- valid passport
-- VISA
-- I-94

Create your Blazer ID
To create
your Blazer ID, please go to
https://chambord.dpo.uab.edu/blazerid.html.

Scroll down to the
Blazerid sign-up/Registration section.

Click I Agree!
If you
have chosen a password that is a recognizable word, rather than a
letter/number combination, it will reject, and you will have to choose
another. They are looking for something like f8l7o2w4e5r (the word flower
with numbers in between the letters). DO NOT GIVE THIS PASSWORD TO
ANYONE! This ID and password get you into our Oracle System where all
vital information about you is kept (payroll information, address, tax
information, etc.).

Set up your direct deposit information
To set up
direct deposit, you will, first, have to have a local bank account. The
idea is that you will not receive a paycheck but will have the money
deposited directly into your bank account. The university will mail you a
summary of your deductions, etc. each month.
When you
have your bank account/checking number, please go to
http://www.uab.edu/adminsystems/. Click on “Oracle HR and Financial
applications (STEPS). When you do, the next page will ask for your Blazer
ID and password. Enter the Blazer ID and password you have chosen. The
next screen should offer you a menu, among them “UAB Self Service
Applications”. Click on this. On the next page, click on the “Manage
Direct Deposit Account” link. This will take you into the section to set
up your direct deposit information.
4.
Complete tax forms online
All
Students: W‑2 and W‑4 income tax forms can be done online through the
Oracle System.
Please go
to
http://www.uab.edu/adminsystems/. Click on “Oracle HR and Financial
applications (STEPS). When you do, the next page will ask for your Blazer
ID and password. Enter the Blazer ID and password you have chosen. The
next screen should offer you a menu, among them “UAB Self Service
Applications”. Click on this. On the next page, click on the “Online tax
form” link. This will take you into the section to set up your necessary
tax information.
Your
paycheck will be processed only after all tax forms, health insurance and
personnel appointment papers have been submitted.

Student Health paperwork
Students
are required to have health insurance. You must go to UAB’s Student
Health Services (SHS) and complete health insurance enrollment forms. If
you currently have insurance, you must take verification of that policy to
the SHS for exemption from UAB’s health insurance. The health insurance
provided for graduate students is VIVA UAB. The department will provide
for single coverage only. Spouse and children are separate and are to be
paid for by the graduate student. For additional information regarding

Schedule and attend Radiation Safety Classes
All new
students are required to attend radiation safety classes. These classes
will be conducted by the Radiation Safety Office (934-4751) at UAB and are
usually scheduled in mid-September. You must contact them to register for
a class.
