CMPh GRADUATE PROGRAM FORMS

Rotation Evaluation Form -- This form is to be used when a student begins a rotation.  The mentor meets with the student to go over expectations of the rotation.  Mentor and student sign the form.  The two meet again when the rotation is complete for the student's evaluation review.  Mentor and student sign the form at the end of the review, and the form is forward back to Jan Phillips, MCLM 702.

Summer Seminar Evaluation Form -- Print out and use this form to evaluate the speakers in the summer seminar series (PHY 751).

UAB GRADUATE SCHOOL FORMS

Committee Formation Form -- This form can be completed online and then printed out.  You must take this form to Dr. Peter Smith for signature and approval.

Application for Degree -- Must be filled out and turned in at the BEGINNING of the term in which you plan to graduate.  Make an appointment with Dr. Peter Smith.  Part of the meeting with her will be the completing of the Application for Degree form.

Dissertation Approval Form -- This is a link to the website where you complete an online request for the approval forms.  24 hours after the submission (weekdays only), you can go by the Graduate School office (HUC 511) and pick up your forms.
